Based on record review and interviews at 10:30 a.m., S4 and S5 failed to ensure C1's diaper was changed when wet or soiled. On 05/12/2026, at approximately 2:09 p.m., O1 picked C1, two-years-old, up from the center and observed C1's diaper was wet and soiled. Although S4 and S5 stated C1 was dry all day, C1's diaper failed to be changed throughout the day, and resulted in C1 departing the center in a soiled diaper. Corrective Action: Effective 05/19/2026, S1 stated S4 and S5 both received write ups, and going forward, staff will follow the center's current diapering technique, which…
