Findings: General: Based on review of documentation, two (2) staff members (DOH: 09/01/20 and 11/14/22) had TB documentation on file that was no longer valid as of 03/10/25 and 09/16/24.

922 KAR 2:120. Section 4. Premises Requirements. (9) Floors, walls, and ceilings shall be smooth, in good repair, and constructed to be easily cleaned. Findings: General: Based on observation in the Preschool #2 Classroom, the surveyor found an area of the wall in the quiet area that had sections of peeled paint. The staff member in charge explained that the paint had peeled off during the removal of signs.

922 KAR 2:120. Section 13. Toys and Furnishings. (1) All toys and furniture contacted by a child shall be: (a) Kept clean and in good repair; and (b) Free of peeling, flaking, or chalking paint. Findings: General: Based on observation, the following was found: Preschool #1 Classroom 1) Excessive black stains on a green area rug in the block center. One-Year-Old Classroom 1) Excessive tape residue on top of the block area shelving unit.

(1) A child care staff member shall complete the background checks required in accordance with this administrative regulation and be found to have no disqualifying offense prior to becoming a child care staff member.
