      Interviewed five (5) staff members. Learned through staff interviews that on 12/ 16/2024 an infant under one (1) year old was being cared for in the Jungle Room toddler room which has an age range of one (1) to three (3) years old. This was during the early morning time during low attendance when children of different age ranges were combined in the same room (Jungle Room).

Learned through interviews and the review of the video that the straps that secure children into one (1) of the highchairs used in the infant room were washed and not reinstalled on the highchair. Staff member1 placed child1 in a highchair with no straps to secure the child in the chair. The highchair was not being used as the manufacturer intended.
