      Self report from director on 2/26/26 stated that video shows caregiver grabbing two year old child by the arms and flipping body over on the cot 5 times. Caregiver is also seen grabbing child's back of head and pushing face into cot twice.

      Director and Assistant director failed to report AD reported that a parent came to the office and put in a complaint. Director and Assistant are to complete Child Abuse and Neglect training by 8/22/25. Please submit certicates of completion to consultant.
