Based on record review/observation at 10:15 a.m., S1 failed to have documentation of the adult staff member not otherwise counted in child-to-staff ratios that accompanied the contractor, O2, at all times while they were at the center when children were present.

Based on record review/observation at 11:30 a.m., a CCCBC-based determination of eligibility for child care purposes from the Department failed to be obtained for each independent contractor, O1 and O2, prior to them being present at the center or performing services. While at the center, the Specialist observed both O1 and O2 performing services with children, unaccompanied by center staff not needed for ratio purposes. Neither O1 nor O2's based determination of eligibility was available for review on the centers CCCBC roster. This could not could corrected during the inspection.
