At the time of visit, observed staff member 1 caring for two (2) children, alone, and in the classroom, unsupervised. This staff has not yet obtained an out-of-state clearance letter and had unsupervised access to children. This was previously cited on 4/13/2026. This is a direct violation of stipulation (L). Stipulation (L) states, "Licensee shall timely submit fingerprint-based background checks, Trails abuse/neglect background inquiry request, and complete out-of-state background check process for any new hire. Documentation of the background checks must be maintained in the staff file at all times."
