    • To Ensure a Safe Exit From the Premises During an Emergency, Early Learning Providers Must Comply with the Following Requirements: (A) Emergency Exit Doors Must…Serious: 110-300-0166(3)

      1. Two emergency exits in the outdoor play area were closed/locked with metal chains preventing children from being able to run out in case of an emergnency.

    • Furniture and Equipment Must Be: (A) Maintained in a Safe Working Condition;110-300-0135(2)

      1. In the infant room the plexy glass is broken and taped on the diaper changing station. 2. Carpets in all the rooms are stained, or dirty. Some of the carpets are ripping and pose trippings hazards. Some of the carpets have edging which is not secured and/or not sealed and the floors can not be cleaned or sanitized. 3. Tiles in all the rooms/hallways and/or restroom are broken, missing pieces, cracked or damaged in a way which prevents the floors from being cleaned disinfected. 4. A couch in the T2 Room is stained on the surfaces.
