It is unclear how the incident happened. The mother of one of the children was not sure since her view was partially blocked by the sensory table. The mother did see the employee yank a child's arm. The second child that their arm was yanked was yanked by the employee who is also the child's mother.

A teacher witnessed a child being hit with a closed fist and reported the incident. The employee was later terminated and OCC received a 1203 delete form on July 17, 2025.
