      Licensing specialists onsite observed children unsupervised by a provider on multiple occasions. On one occasion, two children ages six years and ten years were inside without a provider present. On a second occasion, multiple children of different ages left the premises to ride bikes without the provider. On a third occasion, multiple children under five years of age left the premises to play at the park across from the facility. Children were also left on the premises with a 13- year-old while the provider was off premises retrieving children approximately a block away.
