      Based on interview/record review at 9:30 a.m., the staff daily attendance record failed to include the time of arrival and departure. The Specialist reviewed the attendance records from 3/2/2026, until present, and observed the following: the arrival times failed to be documented for S1 for 3/9/2026 - 3/13/2026; the departure times for S1 failed to be documented for 3/5/2026, and 3/9/2026 - 3/12/2026. Corrective Action: Effective 3/13/2026, S1 will review the attendance policy with all staff to ensure compliance with this regulation.
