Based on observation and interview on 6/06/2025 11:00 a.m., S11 and S12 failed to have the minimum child to staff ratio met at all times on 6/05/2025. At approximately 8:43 a.m., on 6/05/2025, S12 went into the restroom and left S11 in the classroom with nineteen children, ages 2-year-old. The required ratio for children of this age is 10 children per 1 staff person. Corrective Action: Effective 7/01/2025, S1 stated staff were encouraged to call into the office for all restroom breaks to ensure compliance with this regulation.

Staff stated C1 was unsupervised on the playground for less than one minute to three minutes. S4 received a write up during her 90-day probationary period ending 4/31/2025.

S2 notified O1 on 2/25/2025, at 7:50 p.m.

On 2/19/2025, at 3:50 p.m., O2 questioned a bite mark observed on C2's left cheekbone. S11 stated between 2:30 p.m. and 3:30 p.m., while changing diapers, C2 began to cry. S11 did not observe what caused C2 to cry and checked C2 and did not observe any marks on C2 at that time.
