      1507.E. Based on the record review on 12/2/2025 at 2:00 p.m., the center's daily attendance records for visitors failed to accurately reflect when a visitor was on the premises. On 11/12/2025, a DCFS representative conducted an investigation at the center regarding allegations of inadequate supervision and did not sign the visitor's log. S1 stated the DCFS representative walked into the center, directly into her office, and immediately began the investigation. Corrective Action: Effective:

      1915.B.&C.:Based on the interview at 10:00 a.m., S14 and S15 failed to document and immediately notify the parents of an incident that had occurred on 11/11/2025, between 7:15 and 7:20 a.m. C2, a 2-year-old, fell off the bench and hurt her lip during the morning devotional period, but her parents were not notified of her injury. During the afternoon pick-up at 2:54 p.m., the parent questioned S2 about the injury to C2’s lip. S14 and S15 were terminated on 11/12/2025.       .: Based on observations at 1:20 p.m., S2 failed to have sufficient staffing to satisfy the child-to-staff ratios on the premises during rest time and available to assist as needed. During nap time, the Specialist observed 19 children ages 1 -3 years-old asleep with one staff member supervising them in the classroom. There wasn't any additional staff on the premises to help if needed.
