1804.B. 2-3: Based on record review at 11:26 a.m., S1 failed to designate a monitor for each provisionally-employed staff member present at the center. According to center records, S5 (DOH: 8/27/2025) was present at the center and performing duties from 8/27/2025, until 10/10/2025, with a provisionally-employed status without documentation of being accompanied while present at the center. Corrective Action: Effective 1/23/2026, S2 will create a monitoring policy for provisionally employed staff members and ensure compliance with requirements to ensure compliance with this regulation.

Based on record review at 11:45 a.m., a CCCBC-based determination of eligibility for child care purposes from the Department failed to be obtained for S5 and S6 prior to them being present at the center or performing services. Based on record review at 10 a.m., S1 failed to have a Behavior Management Policy that clearly states all types of positive discipline that are used and all methods of discipline that are prohibited. S1 also failed to have the Behavior Management Policy prominently posted and was unable to show parent's signature of receipt.

1509.A.8.c. Based on observation/interview at 7:52 a.m., S1 failed to have a Time Out policy as part of their Behavior Management Policy that clearly states all time out procedures that are used and all that are prohibited. S3 used a prohibited method of time out as a child under age 2 was subject to time out. The Specialist observed C2, 1-year-old, away from the other children in the classroom. S3 stated C2 was in time out. The issue was corrected prior to the Specialist exiting the classroom.

1711.A.B.D.E. Based observation at 7:51 a.m., S1 failed to meet the required child to staff ratio for children of the following ages: 9 children, ages 1-year-old through 2-years-old, with 1 staff, S3. The required ratio for children of this age is 7 children per 1 staff person. The ratio was corrected at 7:55 a.m., when 3 children were moved to S1's classroom.

1713.A.B. Based on observation at 7:50 a.m., children failed to be under supervision at all times. The Specialist arrived to the center and observed S1 exited the kitchen area to another area of the center. When S1 opened the front door to allow the Specialist entry into the center, she was carrying C3, infant. S1 stated she had left C1 unsupervised in the classroom while he was asleep.

C. Based on record review/interview at 9 a.m., S1 failed to have documentation that 1 of 7 staff, S3 (DOH: 2/6/2025), received orientation within seven days and within thirty days of the first day present at the center and prior to having sole responsibility for any children. There is no documentation that S3 has completed LDE Key Training Modules 1 - 3 and DCFS Mandated Reporter Training. The Specialist observed S3 with sole responsibility for 9 children, ages 1-year-old through 2 years-old.

1907. A.1. Based on observation at 7:57 a.m., the manufacturer's restraint device failed to be used when the feeding table was occupied by C1, 2-year-old. This was corrected prior to the Specialist exiting the classroom.
