      Based on record review at approximately 12:45p.m., S1 failed to have documentation that 1 of 3 staff, S1, completed the online child abuse and neglect Mandated Reporter Training provided by DCFS. This could not be corrected during the inspection. Corrective Action: Effective 4/15/2025, S1 stated she would completed the DCFS mandated reporter training by 4/16/2025 to ensure compliance with this regulation.
