Based on record review/interview at 11:00 a.m., S1 failed to have documentation of an incident and that the parent was notified no later than when the child was released on the day of the occurrence. On 2/3/2025, unknown time before lunch, C1, age two-year-old, went up to S4 crying and told her C2, age three-year-old, pushed her. S4 failed to document and report the incident to O1 when C1 was released to O1 that day. S1 was informed on 2/4/2025 at 7:39 a.m., that C1s collarbone was broken following an emergency room visit on 2/3/2025.
