Based on record review and interviews on 4/1/2025, at 10 a.m., children failed to be under supervision at all times. On 3/28/2025, S7 was changing a child and S8 was getting the child's changing clothes out the closet when 2 two-year-olds opened the door to the classroom, walked out of the classroom, down the hall, then out of the center's front door. A parent arrived, gathered the children and began to bring them inside as S2 was coming out of the side door to retrieve them. The time the children were unsupervised was unknown.
