Findings: General: Based on interview and review of documentation, the child-care center did not have a written policy to ensure all staff members take precautions to prevent abusive head trauma and child maltreatment. The director had no knowledge of the written policy.

A statement from a health professional that the individual is free of active tuberculosis; or 2. A copy of negative tuberculin results. Findings: General: Based on interview and review of documentation, a staff member (DOH: 03/13/24) had TB documentation dated 03/20/24. Therefore, the TB documentation was not current.
