Based on record review at approximately 12:00 p.m., S1 failed to ensure the center's daily attendance record for children accurately reflected the children on the child care premises at any given time as 47 children were present and 38 children were signed in on the log. S1 corrected the attendance log prior to the Specialist leaving center. Corrective Action: Corrective Action: Effective 1/8/2026, S10 stated the opening and closing staff person will sign the children in and out of the center each day beginning 1/9/2026, to ensure compliance with this regulation.

Based on record review and observations at 9:15 a.m., the center's daily attendance record for children failed to accurately reflect the children on the child care premises at any given time as 48 children were present and 31 children were signed in on the log. This was addressed and corrected by S2. Corrective Action: Effective 12/2/2025, S2 stated he will complete a count hourly from 8 a.m. - 10:30 a.m., to verify all children present are signed in on the attendance log, to ensure compliance with this regulation.

Corrective Action: Effective 12/2/2025, S2 stated he will speak to staff about the importance of maintaining accurate attendance records and will check daily to verify all staff have signed in, to ensure compliance with this regulation.
