      A five (5) month old infant placed in an unsafe sleep environment. the infant was sleeping in a porta crib with a large bumper pad under the infants head, a child size sheet under the infant's body and another child size sheet covering the infant's body up to the infant’s neck, posing a potential risk of suffocation and Sudden Infant Death Syndrome (SIDS).
