      Based on record review at 12:00 p.m., S1 failed to have documentation that S7 completed the LDE Key Orientation Training Modules 1 training within 7 days of the first day present at the center and modules 2 and 3 within 30 days of the first day present at the center. S7's first day present at the center was 10/20/2025. Module 1 should have been completed by 10/27/2025 and modules 2 and 3 by 11/19/2025. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 1/21/2026, S1 stated S7 is a rehire, so she thought S7 had completed the Modules previously.…

      Based on record review/interview at 12:00 p.m., S1 failed to ensure S7 completed training in Pediatric First Aid and CPR within 90 calendar days of date of hire. S7's date of hire was 10/20/2025, certification should have been obtained by 1/18/2026. This was not corrected prior the Specialist's departure. Corrective Action: Effective 1/21/2026, S1 stated S7 not receiving the training was an oversite. She contacted the main office and they will work on getting S7 scheduled for a training by the end of the month to ensure compliance with this regulation.
