Miles of Smiles Child Care Center

Based on observations at 10:40 a.m., S3 failed to ensure the child to staff ratio was met. Specialist observed S3 supervising six children, age 7-months to 1-year-old. The ratio for this age group is 5:1. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 5/26/2026, S1 stated he will visit with each teacher on Friday, 5/29/2026, and go over the child-to-staff ratio requirements to ensure compliance with this regulation.

Based on observations at 10:40 a.m., S6 failed to ensure the number of children using a room did not exceed the 35 square feet per child requirement. Specialist observed S6 supervising eight 2-year-old children in Classroom #4, which can accommodate 7 children. This was not corrected prior to the Specialist's departure. Corrective Action:

Based on observations at 9:00 a.m., S5 failed to ensure the number of children using a room did not exceed the 35 square feet per child requirement. Specialist observed S5 supervising eleven 2 to 4-year-old children in classroom 3, which can accommodate 5 children. This was corrected prior to Specialist's departure. Corrective Action: Effective 4/8/2026, S2 stated he will ensure there are enough staff present to ensure the classrooms are not over the room capacity and to ensure compliance with this regulation.

C. Based on record review and interview at 1:30 p.m, S1 failed to ensure 4 of 9 staff obtained a minimum of 12 clock hours of continuing education hours per center anniversary year. -S1 had 6 continuing education hours -S2 had 10 continuing education hours -S3 had 4.5 continuing education hours -S5 had 9.5 continuing education hours This could not be corrected. Corrective Action: Effective 1/5/2026, S1 stated she has had staff start completing trainings earlier and they will do more in-person trainings and she will do a progress check in June to ensure compliance with this regulation.

1727.A.&B. Based on record review and interview at 1:30 p.m., S1 lacked documentation that 1 of 9 staff, S2, had a current child abuse and neglect Mandated Reporter Training provided by DCFS. S2's last certificate is dated 6/24/2024. This was not corrected. Corrective Action: Effective 1/5/2026, S1 stated she will create a checklist and attach it to each staff folder to keep track of all certification expiration dates and due dates to ensure compliance with this regulation.

Based on observations at 11:15 a.m., S4 failed to ensure the manufacturer's restraint device on the bouncer was fastened while it was occupied by C1, 6-months-old. This was corrected prior to the Specialist's departure. Corrective Action: Effective 10/30/2025, S1 stated she will have a meeting with the staff on using equipment properly, and she will start doing more walkthroughs in the infant room to ensure compliance with this regulation.

Based on observations, record review, and interview at 9:15 a.m., S4 failed to ensure a daily infant report was available for 3 of 5 children, C6, 6-months-old, C8, 9-months-old, and C9, 9-months-old. This was corrected prior to the Specialist's departure. Corrective Action: Effective 10/30/2025, S1 stated she will make sure the daily infant reports are started when the infants arrive and check periodically throughout the day that it is updated to ensure compliance with this regulation.

Based on observations at 10:15 a.m., S1 failed to ensure no bibs were being worn by a child while asleep. Specialist observed C1, 5-months-old, asleep in a crib with a bib on. This was corrected prior to the Specialist's departure. Corrective Action: Effective, S2 stated she will post in the classroom that no bibs can be worn while children are asleep and will reiterate this with all infant teachers to ensure compliance with this regulation.

Based on observations at 10:00 a.m., S4 failed to meet child-to-staff ratio, as the Specialist observed S4 supervising 8 children aged 3-months-old to 1-year-old, alone. S4 could only supervise a maximum of 5 children; there needed to be another staff member present to supervise the other 3 children. This was corrected prior to the Specialist's departure as S6 arrived at 10:30 a.m. and joined S4.

Based on observations and record review at 10:00 a.m., S3 failed to ensure the number of children using a room was based on the 35 square feet per child requirement, except for dining, sleeping, and other non-routine activities as the Specialist observed her supervising 12 children aged 2-years-old to 4-years-old in Classroom 2. Based on the center's approved floor plan, the capacity for Classroom 2 is 6 children. This was not corrected prior to the Specialist's departure.

Based on observations, record review, and interview at 12:45 p.m., S1 failed to obtain a CCCBC-based determination of eligibility for child care purposes from the department for 1 new staff member, prior to the person being present at the center or performing services as evidence by: S9, (DOH 3/9/2025), being present at the center and supervising 8 3-months to 1-year old children. This was corrected when S1 had S9 to leave the center.
