The ratio for this age group is 5:1. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 5/26/2026, S1 stated he will visit with each teacher on Friday, 5/29/2026, and go over the child-to-staff ratio requirements to ensure compliance with this regulation.

Corrective Action: Effective 1/5/2026, S1 stated she has had staff start completing trainings earlier and they will do more in-person trainings and she will do a progress check in June to ensure compliance with this regulation.

Corrective Action: Effective 10/30/2025, S1 stated she will have a meeting with the staff on using equipment properly, and she will start doing more walkthroughs in the infant room to ensure compliance with this regulation.

Corrective Action: Effective 10/30/2025, S1 stated she will make sure the daily infant reports are started when the infants arrive and check periodically throughout the day that it is updated to ensure compliance with this regulation.

Corrective Action: Effective, S2 stated she will post in the classroom that no bibs can be worn while children are asleep and will reiterate this with all infant teachers to ensure compliance with this regulation.

S4 could only supervise a maximum of 5 children; there needed to be another staff member present to supervise the other 3 children. This was corrected prior to the Specialist's departure as S6 arrived at 10:30 a.m. and joined S4.

This was corrected when S1 had S9 to leave the center.
