Based on record review and interview at 12:15 p.m., although the center has a Behavior Management Policy implemented, S6 (DOH 8/25/2025), used a prohibited method of discipline as C1, age 1-year-old, was subject to physical discipline. On 3/20/2026, at approximately 2:55 p.m., C1 was standing on the railing of the bulldozer climbing toy on the play yard, when S6 grabbed C1 by the left arm, with her right hand, pulling her down from the railing and continued to hold her arm until C1 sat or laid down (toy blocking camera view) on the ground. On 3/20/2026, at approximately 2:55 p.m., S6 picked C1 up by one arm and moved her off the play yard equipment. Corrective Action: Corrective action plan: Effective 4/1/2026, S1 stated all staff has received training on proper lifting of a child to ensure compliance with this regulation.

C1 informed S9 of the incident, however, she did not document the injury or inform the parent.
