Identified through discussion with multiple staff members, different age groups of children have combined in one classroom, especially on Thursdays, so staff breaks can occur. Also identified that toddlers and preschool aged children are combined during outdoor play daily.

Observed no evidence of a general health appraisal form signed and dated by a health care provider in child's file.
