Outside on the preschool playground there were several black plastic playground dividers that were broken.       Confirmed through staff member's statement staff member walked Ch1 down to the nurses office alone. Confirmed through a file review 1 staff member Maggie P was lacking an Out of state background clearance letter.

      Review the attendace sheets for Lady Bug, Panda, Zebra preschool classrooms as well as the nap room and the tranistions were not being completed. Also in the the Dragon Fly, Bear Cubs, and Bummble Bee the attendance sheet were not completed correctly for the tranistions.
