Please submit the Plan of Corrections via the LMS portal within 10 days of the receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 3 of 3 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection.

The following deficiencies were found at the time of the Initial inspection conducted on 8/11/25 and are subject to changes pending programmatic review. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed, but not limited to: 1) Modification application required to be submitted at least 30 days prior to increasing the infant rooms' capacities; 2) Lesson plans are re
