Based on observations at 2:45 p.m., S1 failed to have documentation that S11 completed the DCFS online Mandated Reporter training within seven days of the first day present at the center. S11’s first day present at the center was 09/04/2025 and the DCFS online Mandated Reporter training was completed on 01/05/2026. Corrective Action: Effective 01/27/2026, S1 stated she will require all new staff complete the DCFS online Mandated Reporter training within the first 5 days of the first day present at the center to ensure compliance with this regulation.

Based on record review at 3:30 p.m., S1 failed to have documentation that 1 of 13 staff on premises and accessible to the children have current certification in Pediatric First Aid through training approved by the Department. S13 did not have documentation of this certification. Corrective Action: Effective 01/27/2026, S1 stated she ensure classes are completed in their entirety ensure compliance with this regulation.

      1723.F Based on record review at 1:05 p.m., S1 failed to ensure S11 (DOH 08/26/2025) had current CPR/PFA certification approved by the Department prior to assuming responsibility for any children. The Specialist observed S11 supervising 15 napping children, age 3-years old to 4-years old. This was corrected when S9 entered the classroom at 1:08 p.m. Corrective Action: Effective 01/27/26, S1 stated she will ensure all new staff complete CPR/PFA certification approved by the department within 45 days of their first day present at the center to ensure compliance with this regulation.

Based on observations at 9:15 a.m., S1 failed to ensure the outdoor play space was enclosure with a permanent fence or other permanent barrier in a manner that protects children from traffic hazards, prevents children from leaving the premises without proper supervision, and prevents contact with animals or unauthorized persons. The Specialist was able to open the side gate by slightly pushing on it. The locking mechanism on the gate was not properly working.
