Corrective Action: Effective 01/27/2026, S1 stated she will require all new staff complete the DCFS online Mandated Reporter training within the first 5 days of the first day present at the center to ensure compliance with this regulation.

The Specialist was able to open the side gate by slightly pushing on it. The locking mechanism on the gate was not properly working.
