      At time of inspection, Licensing Specialist observed several children's files that were missing the physician's information and annual updates. Licensing Specialists recommended the facility go through all children's files to ensure all documentation is correct.

      At time of inspection Licensing Specialist observed a loose sheet on a crib mattress in the infant room. This was corrected at time of inspection as the sheet was removed immediately by the Director.
