      The provider had 14 children in care with 1 staff member. The ratio was 10:1 and this placed the provider out of ratio at the onset. The provider arrived later and corrected this issue during the inspection.

      The provider had an expired consent form for staff member PM

    • Caregiver Shall Keep Poisonous or Hazardous Materials That Would Harm Children,

      The provider had soap in the bathroom and lysol spray that were both in the reach of the children. The labels stated to keep out of the reach.
