      Room 2 had a rug placed directly against diapering table. There must be two (2) feet of sanitizable space surrounding diapering table.

      The childs hands shall be washed after diaper changing with a clean, damp, soapy During visit consultant observed staff member in Room 1 did not wash child's hands after diapering.
