      Two outlets on the power strip in one of the classrooms were missing protective covers.

      There was a significant amount of loose gravel on the concrete sidewalk surrounding the outdoor play area - especially on the west side and within the fenced play area. There was a thin metal bar approximately 6 to 8 inches long inserted into the gate handle on the west side of the playground and protruding into the play area. This presents a potential injury hazard.
