      The Licensee was out of compliance with R430-90-10(1) by exceeding the number of allowed children in care. At the time of the inspection, there were 22 children and two caregivers. This rule was previously out of compliance on 8/10/2023 and 8/29/2023.

      The Licensee was out of compliance with this rule by having entrapment hazards on or within the use zone of stationary play equipment. In the outdoor play area, there were gaps between 4 slats of wood on the elevated wooden play equipment that created entrapment hazards.
