Based on observations and record review at 10:30 a.m., S1 failed to ensure documentation of a CCCBC-based determination of eligibility (CCCBC) was obtained for 1 of 33 staff at least once during a five-year period. S28s CCCBC expired on 8/6/2025; she has been present at the center since children returned on 8/18/2025 9/17/2025. S1 had S28 leave the center while the Specialist was present.
