      Identified through the facility's Director statement and two (2) staff members statements that although the incident happened around 12;30PM and CH1 was not using the injury arm throughout the entire day, CH1's parents were not notified of the incident until 4:00PM pick up time. Additionally, identified through two (2) staff members statements that staff did not ask CH1 the reason why CH1 was not using the injured arm, if the arm was still hurting and did not offer CH1 another icepack to place on the injured arm.
