      C.Based on the record review at 10:00 p.m., S1 failed to have at least two staff members trained in medication administration and at least one staff member on the premises during the hours of operation. The Specialist observed that the medication administration certificates for S1 and S2 had expired on January 29, 2026. Corrective Action: Effective 3/13/2026, S1 stated she will set a 30 day calendar reminder prior to the expiration of the training to ensure compliance with this regulation.

      Based on observation at 9:30 a.m. S1 failed to keep the indoor areas hazard-free. The Specialist observed a hole in the classroom ceiling and a hanging ceiling tile. Appears to be caused by water damage. Corrective Action: Effective 3/13/2026, S1 stated she has the ceiling tile repaired to ensure compliance with this regulation.
