Based on record review at 12 p.m., S1 failed to have an application/staff information form that included S11 and S12's date of hire date and first day onsite working with children. The Specialist reviewed the staff attendance and verified S12's first day working was 2/27/2025 and S11's first day working was 4/10/2025. Corrective Action: Effective 4/11/2025, S1 stated she will document the staff dates of hire and first day working on the staff application, to ensure compliance with this regulation.

Based on record review at 2 p.m., S1 failed to provide documentation that 1 of 10 staff, S10, annually completed the online child abuse and neglect Mandated Reporter Training provided by DCFS in 2024. This could not be corrected. Corrective Action: Effective 4/11/2025, S1 stated she will have staff complete the training annually and documentation will be maintained, to ensure compliance with this regulation.

Based on record review at 11:30 a.m., S1 failed to have documentation of S12's CCCBC-Based determination of eligibility available on the centers CCCBC roster at all times. S12's first day working was 2/27/2025 and was added to the roster on 4/11/2025. Corrective Action: Effective 4/11/2025, S1 stated she will add staff to the roster no later than their first day present in the center, to ensure compliance with this regulation.

Based on observation at 11:50 a.m., S2 failed to lift a C1, 4-years-old, using proper lifting techniques and lifted C1 from her cot by her upper left arm. Corrective Action: Effective 4/11/2025, S1 stated she will review this regulation with staff, to ensure compliance with this regulation.

Based on observation on 2/17/2025, at 11:43 a.m., staff allowed children, under age 4, foods that are implicated in choking incidents. Specialist observed 12, 3-year-old, children were served chunks of pork shoulder, larger than what can be swallowed whole, for lunch in S2, S11, and S12's classrooms. This was corrected during the inspection. Corrective Action: Effective 4/11/2025, S1 stated she will make sure all meat is cut properly, to ensure compliance with this regulation.
