      Identified through SM1 that there was 7 children ages 3years to 4years in the Preschool A to assist with potty. When arriving to the Preschool B classroom SM2 stated 7 children ages 3years to 4years of age in the Preschool B classroom with a total of 15 children including the ones going to the bathroom in Preschool B. Attendance verification of 15 children ages 3years to 4years of age were in attendance. In the Preschool B classroom there was 8 children in attendance while the Preschool A classroom had 7 children in attendance.

Facility did report the injury that accored on 3/6/24 within the required time frame, however the other injury that accord on or around 2/21/24 was not reported to the Department.

      Reviewed three staff files and observed Amber to be missing current documentation of Standard Precautions training. Also observed at the time of inspection Kiera to have an expired Standard Precaution training, however Kiera did have a current Standard Precaution training at the time of the injury.

      In the Preschool classroom Kavitha stepped out of the classroom on to the playground closing the door behind her to grab snow. Leaving Vrushali alone supervising 19 children ages 3 years to 3.5 years of age for approximately 30-45 seconds.
