On 6/19/2025, at 11:00 a.m., C1, two-years-old, was left unsupervised in the classroom for less than one minute. S13 was supervising six 3-year-olds, and S24 was supervising five 2-year-olds, while on the playground. S23 allegedly took her two-year-old classroom of children to the restroom, then to the playground as a fire drill began at 8:20 a.m. C1 was left unsupervised in the classroom prior to the class going to the restroom, at an unknown time. C.: Based on record review/interviews on 5/21/2025, at 12:00 p.m., S1 failed to notify the Department and Child Welfare within 24 hours of the following incidents: On 5/14/2025, at 8:20 a.m., S23 left C1, two-years-old, unsupervised in the classroom during a fire drill for more than 10 minutes. The Department and Child Welfare were notified on 5/16/2025; On 2/6/2025 or 2/7/2025, S22 initially notified S1 that S25 allegedly swatted C2's (two-years-old) hand. Based on record review/interviews at 12:00 p.m., on 5/21/2025, although the center has a Behavior Management Policy, S25 (date of hire 1/08/2025) was observed using a prohibited method of discipline. On 2/6/2025 or 2/7/2025 and 4/11/2025, S4, S22, and S23 notified S1 that they observed S25 slap/pop C2, one-year-old, on his hand after he did not follow directions. S25 was terminated on 4/11/2025. Corrective Action: Effective 5/21/2025, S1 stated she has reviewed the center's Behavior Management Policy with all staff to ensure compliance with this regulation.

Based on interviews on 5/21/2025, at 12:00 p.m., while supervising a group of children, S24 failed to devote her time to the supervision of the children, meeting the needs of the children, and in participation with the children in their activities; S24 was observed vaping in the classroom and on the playground while supervising children on several days (dates unknown) by S22. Corrective Action: Effective 5/21/2025, S1 stated she reviewed the center's supervision policies with all staff to ensure compliance with this regulation.

Based on record review/interviews on 5/21/2025, at 12:00 p.m., although the center prohibited the use of tobacco in any form, and the use or possession of illegal substances on the centers premises, on 5/16/2025, S22 notified S1 that she witnessed S24 vaping while supervising children in the classroom and on the playground. S24 then admitted to vaping in the presence of the children during an interview with S1. S24 was terminated on 5/16/2025.
