Reviewed seven (7) child files. The provider uses an enrollment form for children that the parent can sign and date to renew authorizations each year. One (1) child file reviewed had an enrollment form where the last time the parent/guardian signed and dated the form to give written authorization for emergency medical care was on 8/14/2024 and that authorization expired on 8/14/2025.

Reviewed seven (7) child files. At the time of inspection two (2) child files did not contain the provider's enrollment form completed by the parents/guardians. The files did contain health evaluations, immunization records, and documentation that the parents/guardians have recieved and agree to the handbook.
