Based on record review and interviews at 11:30 a.m., S1 failed to obtain a CCCBC-based determination of eligibility for child care purposes from the department for all staff members prior to the person being present at the center or performing services. S2 was hired and began working in the center on 3/2/2026, however, she did not have an eligible CCCBC. S4's CCCBC expired on 2/1/2026. S2 and S4 left the premises immediately.
