      Observed a pack 'n play with an attached cloth canopy. Per the warning label on the canopy, it indicates it can only be used for children 0-5 months. Identified through provider's statement, the child who sleeps in the pack 'n play is 13 months old. No child was in the pack 'n play at the time of visit.

      During walk through of the home it was observed that the lock to the cabinet under the sink was unsecured. There were Clorox disinfectant wipes, Cascade dishwasher liquid, and Dawn dish washing liquid. These items were accessible to children and labeled "keep out of reach of children" posing a potential safety hazard.
