Staff did not have access to phone number for parent of ill child. Licensing specialist indicated to program director that child's parent should be called, and program director indicated she had no ability to access the parent's phone number.

Staff unaware of location of 7-year-old child in care. Child became upset and left the unit. Staff did not pursue the child and were unaware if child remained in the building or left the premises.

Program director did not demonstrate a knowledge of child development when pinching her hands in a child's face as she was telling her to stop talking and when ignoring a crying and distressed child with an earache. Program director never offered comfort to the child and licensing specialist intervened to care for the child.

Staff did not maintain active supervision to protect the safety of a 7- year-old child when child became upset and left the unit. Child's parent was called by staff to come to location and search for her child.

Program director did not demonstrate a knowledge of the licensing regulations when she was unaware of requirements for children's records, location of children's records, and critical incident reporting requirements.

Facility did not submit identifying information for 1 individual as required to complete background checks.

Janitors closet accessible to children when closet door left open in room used for care.

Program staff sitting at table talking and looking at their phones while children were spaced throughout the gym with no activity. One program staff had her back to the room and neither staff were actively engaged with the children. Program staff did not comfort or engage with crying child. Licensing Specialist offered comfort and encouragement to distressed child.

Distressed child located by Licensing Specialist in hallway outside of childcare area. Program staff had no knowledge of childs location.
