      Based on record review at 11 a.m., S1 failed to have documentation that all staff on the premises and accessible to the children had a current certification in infant, child, and adult CPR through training approved by the Department. S4 was present and working following an expired infant, child, and adult CPR certification on 9/30/2025. S4 left the premises at 11:15 a.m., but no class was scheduled. Corrective Action: Effective 10/09/2025, S1 stated she will create a checklist to review expiration of trainings to ensure compliance with this regulation.

      Based on record review at 11 a.m., S1 failed to have documentation that all staff on the premises and accessible to the children had a current certification in pediatric first aid through training approved by the Department. S4 was present and working following an expired pediatric first aid certification on 9/30/2025. S4 left the premises at 11:15 a.m., but no class was scheduled.       Based on record review at 11:15 a.m., the early learning center failed to have documentation of CCCBC-based determination of eligibility on the center's CCCBC roster for S2 and S6. This was not corrected prior to the Specialist exiting. Corrective Action: Effective 10/09/2025, S1 stated she will notify management regarding all staff of any kind being available on center's CCCBC roster to ensure compliance with this regulation.

      Based on observation at 12:19 p.m., the center failed to be free of hazards. The Specialist observed a rat run across the floor underneath a bookshelf in the classroom. The rat was not captured prior to the Specialist exiting. Corrective Action: Effective 10/09/2025, S1 stated she will contact JJ exterminator to come out to service the center to ensure compliance with this regulation.
