B.3.C.2.: Based on record review/interview at 10:45 a.m., S1 failed to notify the parent immediately and DCFS Child Welfare within 24 hours of the following critical incident: On 11/14/2024 at 10:35 a.m., O2 observed C1, age 2 years old, being picked up off the ground by one arm by S21 moving the child away from a shelf unit because the child was attempting to flip the unit over O1, C1s mother, was notified on 11/14/2024 at 3:11 p.m. Child Welfare was notified on 11/18/2024.

      Based on record review/interview at 1:30 p.m., although the center established a behavior management policy, S1 failed to ensure the behavior management policy was followed on 11/14/2024 at 10:35 a.m. when S21 (DOH: 9/16/2024) yanked C1, 2 years old, by the arm two times away from a book shelf he was pushing and kicking, placing C1 on his butt on the floor. S21 is still employed and present at the center.
