Based on record review at 12:15 p.m.,S1 failed to maintain accurate and complete records of sign in/ sign out attendance for independent contractors to include the departure times. The Specialist observed O1 signed into the center on 4/2/2026 at 10:30 a.m., but she did not sign out. There was no signature of staff member whom accompanied the contractor while providing services in the center. Corrective Action: Effective 4/8/2026, S1 stated when contractor's arrive she will initial daily on attendance log, which will represent visual check, to ensure compliance with this regulation.
