Upon the surveyor's arrival, a 10 month old infant was asleep in a bouncy seat with a fleece blanket surrounding the infants head and chest (face uncovered) and was not moved to a playpen. (Corrected upon the surveyor's request.)

During nap time, six cots were not separated by 24 inches.

Each child under 12 months of age (two children, 10 months of age) were placed on a bobby pillow for nap time. (Corrected upon the surveyor's request.)

One of three playpens does not have a fitted sheet.

A 10 month infant was placed to sleep in a play pen and the provider folded a fleece blanket into four layers and lay the blanket on top of the infant's chest, not tucking the blanket along the sides and foot of the mattress.

Tornado drills have not been recorded since December 2021.

Fire drills have not been recorded since November 2021.

Disaster plans do not include procedures for fire.

Current immunizations are not on file for five family dogs and one family cat (all expired 12/6/22).

Upon the surveyor's arrival at 11:50am, the license capacity was not maintained when one provider was present with 10 children as follows: three children 10 months to 16 months; five children 18 months to 5 years (over by two children); and two children 5 years to 11l years. Current pediatric first aid and pediatric CPR certifications are not on file for two providers when each certification expired on 12/2022.

One 3-tier swing that can be used while standing is not anchored in the ground and large rocks, large tree stumps above ground and a tree are located in the use zone. A 10 month old infant was napping and sleeping in a room separate from the provider and the door was closed.
