Based on record review/interview on 4/9/2025, S1 failed to provide documentation that all staff annually completed the online child abuse and neglect Mandated Reporter Training provided by DCFS. S5 (DOH 3/8/24) and S13 (DOH 10/23/2023) did not have the required training. S5 completed her most recent training on 4/3/2024, which expired on 4/3/2025. The last date of completion for S15 was unknown, because S1 was unable to locate the previous certificate of completion.
