The step stool near the Toddler 4 classroom sink was 16 inches tall with two large steps and no side handrails which was not safe for toddler age children to use without assistance. CH1 was using the step stool to reach the sink and fell.

No one was sitting with the children in the school age classroom during lunch. Three staff were in the classroom with two serving and one putting out nap cots. When classroom teacher was advised by Director to sit with the children, she began taking role and completing paperwork but not interacting with the children other than through custodial/control speech (Stop this. Stop that. Settle down guys. Don't do that.) When aides finished serving one left the classroom and the other stood back from the table and the children but did not sit with them or engage with them.
