Based on record review and interviews at 10:00 a.m., S1 failed to ensure that staff were supervising children at all times. During nap on 03/18/2026, at around 12:40 p.m., S37 left the classroom and went into the hallway with C1, two years old, leaving 12 children (2 one-year-olds, and 10 two -year-olds) unsupervised in the classroom. S1 discovered no other teacher was present in the classroom while S37 was in the hallway. The Specialist reviewed camera footage from the incident and footage showed that the children napping were left unsupervised for approximately 3 minutes.…

1509.A.8.a.&b.: Based on record review and interviews at 11:30 a.m., although the center has implemented a written behavior management policy that prohibits children being subjected to physical or corporal punishment, S38 and S39 used the following prohibited methods of discipline: On 10/14/2025, between 2:11 p.m. and 2:12 p.m., S38, slapped C1, one-year-old, in the face, snatched a block from C2, one-year-old, and hit him in the face with the block, picked C4, one-year-old, up by the arms and ran around the classroom holding him in her arms. C.: Based on interviews on 10/16/2025, at 10:00 a.m., S1 failed to notify the Department within 24 hours of the following critical incident allegation: On 09/18/2025 C1, 1-year-old, fell, causing her to hit her head on a door frame and two staff stating the child passed out. Corrective Action: Effective 10/16/2025, S1 stated she will require staff to submit written statements, to verify all information and retraining on CPR first aid emergency preparedness with drills, to ensure compliance with this regulation.

Based on record review/ interview on 10/16/2025, determination of eligibility for child care purposes from the department was not obtained for independent contractor, prior to the person being present at the center or performing services. The Specialist observed O3 signed into the center and not accompanied by a staff member. The Center did not have documentation of the independent contractors CCCBC based determination. 1713 A.B.C.: Based on record review/interviews on 07/03/2025, at 11:30 a.m. S6 and S9 failed to supervise children at all times. On 07/02/2025, at 10:17:44 a.m., while transitioning to the indoor play area S6 and S9 left C1 (4-years-old) in the classroom unsupervised for 26 minutes. C1 was found by O2 at 10:43:11 a.m. and retrieved from the classroom by S6 at 10:43:43 a.m. Corrective Action: Effective July 7, 2025, S1 stated that between her S2, Designee, and S37, Designee, each classroom will be checked three times a day and signed off on to ensure compliance with this regulation.

1719 A.-C.: Based on record review on 07/03/2025, at 11:00 a.m., S1 failed to ensure staff completed the following orientation training within seven calendar days of the first day present at the center, and before assuming sole responsibility for any children: S16's first day present at the center was 05/27/2025, the DCFS online Mandated Reporter Training was not completed as of 07/03/2025. Based on video review at 12:30 p.m., S4 and S21 failed to devote their time to the supervision of the children, meeting the needs of the children, and participate with the children in their activities while under their supervision. On 4/01/2025, around 9:15 a.m., while on the Frog Playground, C1, three-years-old, and several other children, names and ages unknown, were fighting near the fence. Based on record review interviews at 12:15 p.m., although the center has implemented a Behavior Management Policy that prohibits physical punishment, S23 (DOH 1/23/2025) was observed on the centers video recording using a prohibited method of behavior management. On 02/14/2025, at approximately 2:31 p.m., S23 grabbed C3 (2-year-old) roughly by both forearms, pulling him as he jerked away to discipline him for moving a chair. C3 fell to the floor while S23 pinned both his arms down, holding his wrist and standing over him as C3 continued to kick his feet.

: Based on record review and interviews at 12:00 p.m., S13 and S35 failed to devote their time to supervising their classroom of 16, one, and two-year-olds. The Specialist observed S13 and S35 sitting in chairs at the table with four children and the remaining children playing in another area of the classroom, out of S13 and S35s view on the center's camera footage. S13 stated they were completing artwork, and S35 wanted to sit at the table with her and help with arts and crafts instead of monitoring the other children. S35 was terminated on 2/12/2025.

1911. D.: Based on observation on 2/20/2025 at 10:45 a.m., S1, S2. and S4 failed to ensure awake children did not remain in a crib. S32 and S34 failed to ensure awake children did not remain in a crib, baby bed, swing, high chair, carrier, or playpen for more than 30 consecutive minutes. The Specialists observed three infants awake in their cribs for 30 minutes while S32 sat in the rocker holding another infant, and S34 washed her hands at the sink.

I.J.: Based on record review interviews at 11:30 a.m., S1, S2, and S4 failed to ensure that staff members adhered to proper techniques for lifting a child on the following dates:On 02/12/2025 at 9:31 a.m. and 9:52 a.m. S35 failed to use proper techniques for lifting children. According to video footage, C1 (2-year-old) was observed being lifted by one arm, placed on S35's hip, and then placed in a chair. 1711.A. B. D.: Based on interviews at 12:30 p.m., S1, S8 and S37 failed to meet the required child to staff ratio for one-year-old children on 12/18/2024. S6 stated on 12/18/2024, at 9:13 a.m., she was supervising 8 or 9 children, one-year-old, while on the Caterpillar Playground. The required ratio for children age one-year-old is 7:1.
