      C.1.2.: Based on record review and interview on 2/27/2026, at 11 a.m., S8 failed to notify the Department and DCFS within 24 hours of the following critical incidents: On 10/30/2025, at approximately 8:03 a.m., C1, 7-year-old, exited the building without staff supervision. A parent found and returned C1 inside the building to S8 at 8:10 a.m. On 10/31/2025, between 2:30 p.m. and 3:30 p.m., C2, 11-year-old, climbed onto and fell from an infant swing, resulting in a broken arm requiring medical care.       Based on observation and record review at 11:15 a.m., S8 failed to ensure an accurate daily attendance record for independent contractor’s that included O1’s first and last name, arrival and departure times, name of staff member that accompanied O1, and purpose of the visit. The Specialist observed O1 unaccompanied in a classroom with a one-year-old child. O1 was not signed into the center.       Based on observation and record review at 11:15 a.m., S8 failed to maintain a daily attendance record for all visitors to include the visitor’s name, date of visit, arrival and departure times, and the purpose of the visit. The Specialist observed O2, O3, and 9 additional parents were present and not signed into the center’s attendance record.       Based on Record Review at 11:30 a.m., S8 failed to obtain a CCCBC-based determination of eligibility for child care purposes from the department for each volunteer, S6, O2, and O3. The Specialist observed S6, alone in a classroom supervising six children, ages 7 to 12 years old, O2 and O3 were observed walking unaccompanied throughout the center.
