Corrective Action: Effective 04/16/2025, S1 stated she will check the visitors daily attendance daily to ensure visitors are signing in and out to ensure compliance with this regulation.

The Specialists observed the play yard to have a broken plastic blue slide and two broken soccer nets. Corrective Action: Effective 4/16/2025 S1 stated she will remove any broken play yard equipment and have staff check the play yard equipment daily before children go outside to ensure compliance with this regulation.

Corrective Action: Effective 04/16/2025, S1 stated she will have staff check the outdoor play space every morning to ensure compliance with this regulation.
