Based on record review at 10:50 a.m., S1 failed to maintain documentation of a daily attendance record for visitor's to include departure times for the following individuals on the specified O1 and O2 on 2/3/2025, O3 on 3/18/2025 , O4 on 3/26/2025, O5 on 3/27/2025, O6 on 4/1/2025, O7 on 4/3/2025, O8 on 4/3/2025 and O9 on 4/11/2025. Corrective Action: Effective 04/16/2025, S1 stated she will check the visitors daily attendance daily to ensure visitors are signing in and out to ensure compliance with this regulation.

      1901.G.H. Based on observation at 10:00 a.m., S1 failed to ensure all of the center equipment used by children was maintained in a clean and safe condition and in good repair. The Specialists observed the play yard to have a broken plastic blue slide and two broken soccer nets. Corrective Action: Effective 4/16/2025 S1 stated she will remove any broken play yard equipment and have staff check the play yard equipment daily before children go outside to ensure compliance with this regulation.

Based on observations at 10:00 a.m., S1 failed to have the outdoor area free of hazards as the Specialists observed a broken plastic blue slide and two large screws sticking out of a plastic slide on the outdoor play yard. Specialist observed a moldy stain located on the wall in classroom 6 were children were observed. Corrective Action: Effective 04/16/2025, S1 stated she will have staff check the outdoor play space every morning to ensure compliance with this regulation.
